Office Visit Registration Form
Wen you visit our office, please be sure to bring your passport and your medical insurance card. This protects your insurance company and you from fraud. If you haven't visited us within the past year, or if your previous registration information has changed since you last visited, you will need to complete a new registration, in one of the following manners:
> Print and fill out our patient and medical information sheets, then bring them with you to your appointment, or
> Arrive approximately 15 minutes before your scheduled appointment in order to complete this necessary paperwork.
